I hold a current Amateur Radio Certificate of Proficiency
My details will be checked with the ACMA and I authorise the ACMA to provide information to Scouts Australia, Victorian Branch in order to verify my license status.
If I am allocated a callsign for the event; That my current licence limitations still apply and I will not operate outside those limitations.
If I am allocated a callsign for the event; The allocation of the callsign will be recorded and may be provided to the ACMA on demand.
I will be responsible for the safe setup and operation of the equipment and will ensure that only equipement safe for supervised use around children will be operational.
If I am not an Adult Member of Scouts Australia, Victorian Branch, I will ensure that there is an Adult Leader in my presence whenever there are youth members around.
I have read and agree to abile by the Code of Conduct for Adults in Scouting.
I have provided the details of my Working with Children Check and agree to carry and show the WWC Card when requested.
If I am attending a site overnight I must complete a Personal Information Record and Health Statement.
